Acknowledgement of Risk and Personal Responsibilities
All GW Nursing students are required to sign the GW Nursing Clinical Programs Acknowledgement of Risk and Personal Responsibilities Form prior to being cleared to enter the clinical setting.
By signing the form, students acknowledge that they wish to voluntarily participate in direct patient care clinical/practicum hours, agree to abide by policies and procedures put forth by the clinical/practicum site, and understand the public health issue that currently exists due to COVID.
